First Time Instructions

Upon registering for the Payment Portal for the first time, you will be asked for the following:

Account Number:
Last Name:
Date of Birth:
Member ID:

Once you have created your account, just use your Patient Account number and password that was created upon registration to login to for your payment to Hometown Family Care with the online link button above. If you have any questions, please contact us.
